Option 1

For Regular season we only need 1 POR document- AS will require 3

Document must include:

First & Last Name
Address (must match account)
Date - must be dated AFTER 2/1/2024 but BEFORE 2/1/2025

Official School Form
Please fill in as much information as possible prior to printing


Please make sure attachments are clear and legible

When saving/adding/emailing attachment - keep at original size, DO NOT REDUCE
